Fourteen modules to ICAO Annex 19

Occurrence, hazard and continuous-improvement reports arrive through a form that can be submitted anonymously, then move through a five-phase case — Inbox, Triage, Investigate, Pending Close, Closed. Risk is assessed on pre- and post-mitigation matrices, findings are tracked to closure, and the safety review board gets a PDF pack rather than a folder of screenshots.

Ask what the rule actually says. Chuck reads a curated corpus of aviation regulation — not the open web — and returns the passages behind the answer: rule reference, source document, and the amendment that passage belongs to. Where the corpus does not support an answer, it says so instead of inventing one.
